• Our fees are designed to be as straightforward as possible, so patients are never surprised with a medical bill from our office.

• These fees are applicable to patients who do not have insurance or have insurances we are out-of-network with i.e., Aetna, Blue Cross, United Healthcare, Medicaid, Out of State etc.,

• We accept all major credit cards, check, cash, health savings account (HSA) and flexible spending accounts(FSA).

• We can provide a “super bill” so you can submit the claim to your insurance.

• We offer 20% discount if the patient has Military | Healthcare | Police | Firefighter | Teacher background.

1. Consultation (Initial visit): $400

2. Follow- Up: $200

3. Telehealth (Video | Phone): $200

4. Ultrasound Guided Peripheral Nerve Block (One Side | One Nerve): $400

5. Ultrasound Guided Sympathetic Block: $500

6. Ultrasound Guided Trigger Point Injections: $300

7. Ultrasound Guided Joint Injections (Knee | Hip | Shoulder) : $300

8. Sacroiliac (SI) Joint Injection: $500

9. Epidural Blood Patch: $600

10. Cervical Epidural Steroid Injection: $500

11. Lumbar Epidural Steroid Injection: $500

12. Medial Branch Blocks: $500

13. Radiofrequency Ablation: $600

14. Cryoneurolysis of Peripheral Nerve: $1,200 (One nerve Only, every additional nerve will be $1,200)

15. Peripheral Nerve Stimulator Implantation: $2,500

16. Peripheral Nerve Catheter for 7 days: $2,500

17. IV Lidocaine Infusion: $500

18. Botox Injections: $400

19. Botox: 50 Units= $600; 100 Units =$1,200; 200 Units =$2,000

20. Nitrous Oxide (laughing gas) conscious sedation: $200

21. NO Show or Cancellation <24 hours: $200

    NOTE: If the procedure is performed at an out patient surgery center, the fees charged by that center are independent of our practice and are in addition to the professional fees we charge.
